Connected Resources
In the Pathways program, our Community Health Workers look at everything a family needs to stay healthy, including access to house, healthy food, medical care, health insurance, safety, transportation, education, employment, and other services. We provide home and community-based services based on individual needs and work alongside family members to help lower health risks and see positive results.
The Carver Center for Families (CCF) in Georgetown, Texas is a family resource center (FRC) that nurtures the health and well-being of children, youth, and families, and builds on the strengths of our community’s parents, caregivers, and residents, as well as the organizations that support them.
